Patient responsibility: 20% coinsurance and annual Medicare deductible when applicable

Important Requirements

Patient responsibility: Bring all Workers' Compensation / No Fault Insurance and Attorney information at the time of your initial visit.

HMO Requirements

Patient responsibility: Provide insurance information, copy of insurance card(s) and referral

Important: If referral is not available at time of visit, payment is required. Reimbursement available upon valid referral (minus co-payment, coinsurance and/or deductible)

Patient Financial Responsibility

As a courtesy to our patients, we will submit claims to your insurance company on your behalf. However, please be aware of the following responsibilities:

  • Co-payments: Due at the time of service for each visit
  • Deductibles & Coinsurance: You are responsible for any deductibles or co-insurance as determined by your insurance plan
  • Non-covered services: Some services may not be covered by your insurance plan
  • Referrals (HMO): Required for HMO patients - must be presented at time of visit
  • Insurance updates: Notify our office immediately of any changes in insurance or employment status
  • Medical necessity: If insurance deems services not medically necessary or non-covered, the balance becomes your personal responsibility

Important: If your bill becomes past due/delinquent, you become fully responsible for any late fees and/or collection fees additionally incurred.

Private Insurance & Self-Pay Options

Private Insurance: If you are not covered under any of the insurance plans that Dr. O'Hanlon participates with (or do not have insurance), payment for the initial consultation/office visit, x-ray(s), supplies and/or any services (surgical or otherwise) are required on the day of your appointment.

We will mail you an itemized receipt to submit for reimbursement to your insurance company if requested.

Self-Pay Patients: We believe that quality podiatric care should be accessible to all patients, regardless of insurance status, and will work with you to find an affordable solution for your care.

Payment Methods

Co-payments and payments for services not covered by your insurance are to be paid at the time service is rendered.

Medical Information Release & Assignment of Benefits

By receiving services, you authorize Dr. John O'Hanlon to release any information acquired for purposes of treatment, payment and healthcare operations. You also authorize payment(s) to go directly to Dr. John O'Hanlon, D.P.M.

HIPAA Privacy Practices

As required by HIPAA, you will receive a copy of our Notice of Privacy Practices. Your consent acknowledges understanding of how we use and protect your health information for treatment, payment, and healthcare operations.
